How Long Should Tooth Sensitivity Last After a Filling?
Some sensitivity after a filling is normal and expected — but how much, and for how long, before it's worth a call? Here's the real timeline, and the signs that point to something needing a closer look...

Mild, self-limited sensitivity and soreness are the norm after a routine filling, and they resolve over hours to a few days for most people. About 78% of patients report some discomfort within the first 24 hours — most commonly transient sensitivity to cold, which is mild in roughly 78% of cases and fleeting (gone within seconds) in about 94% of those affected. Sensitivity to sweets, notably, isn't typically a feature of a routine restoration — if that's what you're noticing, it's worth mentioning at your next visit. Separately from sensitivity, postoperative pain of some kind is reported after roughly 36% of restorations; it's usually low-grade and drops substantially by days 4–7 compared with the first 24 hours.
Why It Happens at All
Placing a filling, especially a tooth-colored composite, involves removing decay, etching the remaining tooth structure to help the filling bond, and building up new material in layers. Each of those steps can temporarily irritate the nerve inside the tooth: the etching process opens up microscopic channels (dentinal tubules) that connect to the nerve, cavity preparation itself can generate a small amount of heat and dehydration, and the composite material shrinks slightly as it hardens, creating a small amount of stress in the tooth. None of this means anything went wrong — it's simply part of what a filling does to the tooth structure while it's placed.
What's Expected and Normal
Numbness first. The local anesthetic wears off over one to several hours after your appointment — avoid chewing, hot foods or drinks, and try not to bite your cheek, lip, or tongue until sensation fully returns. Soreness at the injection site itself is actually the second most common complaint after a restorative procedure, right behind sensitivity in the tooth, so don't be surprised if the area where you got numbed is a little tender too.
Cold and thermal sensitivity. Depending on how deep the cavity was and how the filling was configured, postoperative thermal sensitivity shows up in roughly 5–26% of composite fillings — on the wider end of that range for deeper preparations. It's typically mild and fades over days to a few weeks as the nerve settles down from the irritation of the procedure itself.
Bite tenderness. Mild tenderness when chewing is common, especially with deeper fillings. Interestingly, how much it bothers a patient (measured by how often they reach for a painkiller) tracks with how deep the filling was — specifically once a restoration goes past roughly 3mm in depth — rather than simply how large the filling is across the tooth's surface. A big but shallow filling and a small but deep one aren't equally likely to ache; depth toward the nerve is what matters most.
Timing. Across the board, discomfort of any kind is greatest in the first 24 hours and declines steadily over the following week, which matches the pattern for sensitivity specifically, discussed below.
The Normal Sensitivity Timeline
Mild sensitivity to cold or pressure in the first few days after a filling is common and expected. For most patients, it steadily improves and is mostly or fully resolved within one to two weeks. Deeper fillings — ones placed closer to the nerve, which need more tooth structure removed — can reasonably take three to four weeks to fully settle. A clinical trial that tracked sensitivity scores at 24 hours and again at one week found a clear pattern: even without any special desensitizing treatment, sensitivity dropped substantially from day one to day seven, and by one week, there was no longer a meaningful difference between treated and untreated groups — sensitivity naturally trending down with time is the expected pattern, not the exception.
Managing It Along the Way
Pain relief. An over-the-counter NSAID (like ibuprofen) or acetaminophen, taken around the time of treatment and for the first 24 hours, controls most of this discomfort for most patients. Between the two, NSAIDs tend to be the more effective option when there's no medical reason to avoid them (certain kidney conditions, blood thinners, or a history of stomach ulcers, for example — ask us if you're unsure). The American Dental Association's 2024 evidence-based guideline on acute dental pain doesn't recommend opioids for routine restorative work like a filling; over-the-counter options are the appropriate first choice.
Desensitizers. Some dentists apply a desensitizing agent during the filling procedure itself specifically to reduce this early sensitivity — research comparing these products shows a real, measurable reduction in sensitivity at the one-day mark, though by one week, sensitivity tends to converge regardless of whether a desensitizer was used, since it settles on its own either way. If you're sensitive after a filling, a desensitizing toothpaste (containing potassium nitrate or stannous fluoride) used for a few weeks, along with simply giving it time, is usually all that's needed.
Eating. Wait until the numbness fully resolves before eating anything. Once feeling is back, favor softer foods for the rest of that day, and skip very hot or very cold items for a day or two if the tooth feels sensitive. As above, a lingering sweet-specific sensitivity isn't part of the typical picture — most patients don't notice sugar as a trigger the way they notice cold.
When to Seek Dental Review
A few patterns are worth flagging rather than waiting out:
- Persistent or worsening pain beyond about one to two weeks, or pain that's spontaneous (happens on its own, without a cold, hot, or pressure trigger) or specifically triggered by heat rather than cold — together, these patterns can suggest irreversible pulpitis or pulp necrosis, which may need root canal treatment rather than time alone to resolve. - Pain centered on a persistently sensitive margin (the edge where the filling meets the natural tooth) can point toward incomplete bonding, microleakage, or, further down the line, secondary decay forming underneath or beside the restoration — all reasons to have it checked rather than assume it's ordinary healing.
- Swelling, or pain severe enough to disrupt sleep or daily activities — this goes beyond the expected discomfort range and warrants being seen soon.
Most of the time, a high bite (the filling sitting a fraction too tall) is a simple, five-minute adjustment, and true nerve involvement requiring further treatment is the less common outcome — but the only way to know which pattern you're in is to have it checked rather than assuming either extreme. Yes — some tenderness or sensitivity as the numbness fades and you resume normal biting and temperature exposure is expected and not a cause for concern on its own. What matters more is the trend over the following days and weeks, not the very first few hours.
Composite fillings bond directly to the tooth structure via the etching and adhesive process described above, which is part of why they're more commonly associated with some early postoperative sensitivity than older-style metal fillings, which don't rely on the same bonding mechanism. This doesn't mean composite is a worse choice — the trade-off is offset by composite's cosmetic advantage and more conservative tooth preparation.
Occasionally, yes — a filling placed very close to the nerve can, in a minority of cases, lead to the nerve becoming irritated enough over time to eventually need root canal treatment, even if the initial postoperative course seemed uneventful. You don't need to avoid cold foods and drinks entirely, but it's reasonable to expect some discomfort with them for the first couple of weeks, and many patients find it more comfortable to favor room-temperature foods on that specific tooth during that window.
An over-the-counter NSAID like ibuprofen, or acetaminophen if NSAIDs aren't right for you, is the appropriate choice for routine filling discomfort — this matches current ADA guidance on acute dental pain, which doesn't call for opioids for procedures like this. If over-the-counter medication isn't controlling the pain, that's itself a sign worth mentioning at a follow-up rather than a reason to seek something stronger on your own.
