Fillings are routine, which is exactly why it can feel odd to question one — but being told at a new-patient visit that you suddenly need several fillings at once, especially for cavities you weren't told about at your last checkup, is one of the more common reasons patients seek a second opinion.

Diagnosing a Cavity Isn't Always Black and White

Early, shallow cavities are sometimes borderline calls rather than obvious ones, and research on this has found real variation between examiners. A study comparing dental students' caries assessments and restorative treatment decisions against normative standards found significant misdiagnosis and a meaningful degree of overtreatment proposed for teeth that didn't clearly need it — a reminder that “does this need a filling” isn't always a single obvious answer, even among trained clinicians.

When a Second Opinion Makes Sense

It's worth asking for one if several fillings are recommended at once with little explanation, if a tooth flagged for a filling isn't causing you any pain or sensitivity, or if you're simply unsure why a particular tooth needs treatment now rather than being watched. Very early lesions can sometimes be managed with fluoride and better home care instead of being drilled and filled right away — it depends on how deep the cavity actually is.
