We Accept Guardian — Here's How It Works

We accept Guardian PPO plans, including DentalGuard Preferred, that allow out-of-network coverage. We're not on Guardian's contracted in-network provider list, which means we don't operate under Guardian's negotiated fee schedule — instead, we file your claim on your behalf as a courtesy, and Guardian reimburses you (or in some cases us directly, if assignment of benefits is available on your plan) according to your plan's out-of-network benefit level. You're responsible for the difference between our fee and whatever Guardian classifies as the allowed or “usual, customary, and reasonable” amount for that out-of-network benefit.

One important note: if your specific Guardian plan is a DMO, DHMO, or other closed-network plan with no out-of-network benefit at all, we generally can't accept it, since there'd be no way for you to get reimbursed. Please note that all plans, in-network or out-of-network, can still have yearly maximums, deductibles, waiting periods, and missing-tooth clauses that limit what's actually covered — your specific plan booklet or HR benefits contact is the most reliable source for those details.
