Root Canal or Extraction? Getting a Second Opinion First

Told a tooth needs to come out? Extraction is permanent — a second opinion, ideally from an endodontist, is one of the more worthwhile ones to get before deciding.

Root Canal or Extraction? Getting a Second Opinion First

By Dr. Boris Zusin · Published August 22, 2026

Of all the situations where a second opinion is worth the extra visit, being told a tooth needs to come out is near the top of the list. Extraction is permanent — once the tooth is gone, that decision can't be undone — while a root canal, in many cases, can save the natural tooth instead.

Why This Decision Deserves an Extra Look

A tooth may genuinely need extraction if it's cracked below the bone line, has severe surrounding bone loss, or is too damaged to support a restoration afterward. But not every tooth that's been flagged for extraction is actually beyond saving, and the equipment used to make that call matters. Endodontists work with operating microscopes and imaging built specifically for evaluating canal anatomy and root fractures — tools that can sometimes reveal options a general exam doesn't.

Where We Fit Into This

We refer root canal treatment to endodontists we know and trust rather than attempting it in-house, which means a second opinion on a root-canal-versus-extraction question fits naturally into how we already practice — we can coordinate that evaluation directly. Once a root canal is complete, we handle the restoration, typically a crown, so nothing falls through the cracks between offices.

Cost Is Part of the Calculation Too

It's worth knowing that extraction plus replacement — an implant or bridge — typically costs more over time than a root canal plus crown, which is one more reason it's worth confirming extraction is truly necessary before proceeding.
